What Information We Collect and How We Use It
We want you to understand the kinds of information we collect from you and what we do with this information.
Personal and Health Information
We require you to register, acknowledge receipt of this Policy, and agree to the Agreement to use the Site. To help us tailor certain areas of the Site to your needs and interests, we will ask you to provide some personal information when you complete the registration form. In addition, you will be asked to supply personal information, including personal health information, when you enroll in personalized tools, trackers, and services available on the Site. When you are requested to register or enroll in certain areas of the Site but choose not to provide this information, you may not have access to the tools, trackers, and services in these areas and, if you do, such tools, trackers, and services may not be optimized for your use. We recognize the importance of protecting your personal information, including your personal health information, and keep that information stored in our secure database. We may also ask you to supply demographic information such as age, gender, and areas of interest, and in addition, we may collect information regarding the "IP address" that is used to identify your computer on the Internet and information obtained by tracking your usage of our Site. We use this information to improve our Site, by measuring interest, traffic, and usage patterns.
Changing Your Personal Information
You may change or delete your personal information by going to the appropriate administrative section of the Site and submitting the appropriate changes. If you provided personal information within a particular tool or tracker, you can change or delete that information by visiting those tools and submitting the appropriate changes.
If the third party sponsor of your membership (e.g. your health insurer) terminates its relationship with Alere, your personal information will be removed from our database. If at any time, you wish to permanently remove any personal information from our system, please contact us directly at the e-mail address provided at the end of this Policy. Otherwise, Alere may maintain your information for the period of time required by applicable law or destroy it or return it to the third party sponsor of your membership.
We Will Use Your Personal Information to Contact You
We will use your personal information to contact you, if necessary, or, if you choose to receive e-mail from us, to send you newsletters and to inform you about special offers or events that might interest you. In the event the e-mail address you provide to us is your work e-mail account or is otherwise provided by your employer, you expressly acknowledge that your employer may have access to any e-mails sent to that address in accordance with your employer's policy on monitoring employee e-mail. E-mails that we send to you may contain health and wellness information that we think will beuseful to you based on the information you provide us on the Site, as well as personal health information.
We may share information we collect from you with the sponsor of your membership (e.g. your health insurer) and with individuals and entities involved in your health plan and/or your health care in accordance with all applicable laws. Uses and disclosures of Protected Health Information outside of treatment, payment, and other operational needs of such parties are not permitted under law without your prior consent. In addition, Alere reserves the right to de-identify and aggregate data, and to use such de-identified, aggregated data in accordance with all applicable laws. If your membership is sponsored by your health insurer or another third party, we may share information with that party as permitted under applicable law. For example, we may report to your employer the number of its employees using the Site or the frequency of their collective use. If you provide us with your e-mail address and you have agreed to receive e-mail from us, you always have the option to change your mind about receiving e-mail from us. Simply follow the unsubscribe directions in the footer of our newsletters and promotional e-mails.
If your health insurer has contracted with us to provide services, we may provide personal and/or health information to your health insurer and to certain individuals designated by your health insurer, in compliance with the Employment Retirement Income Security Act of 1974 ("ERISA") and the Health Insurance Portability and Accountability Act of 1996 ("HIPAA"), to have access to such information in order to perform certain administrative functions or to carry out treatment, payment or health care operations on behalf of the health benefit plan. Your employer or health insurer may elect to offer financial incentives to you to encourage your participation in the services offered through the Site. In such event, your employer or health insurer may be informed if you qualify for the financial incentive based upon your participation.
We have security measures in place to protect against the loss, misuse, and alteration of the information you provide us. We use SSL (Secure Socket Layer) to encrypt and password protect all information transmitted to or from the secure areas of the Site. We have implemented and maintain administrative, physical, and technological safeguards that we believe reasonably and appropriately protect the confidentiality, integrity, and availability of all personal information, including protected health information, submitted by you through the Site.
Although we use reasonable efforts to protect your privacy on the Site as described in this Policy, you should keep in mind that if you give others access to your computer, your password or disclose personal information online, it is possible that, despite our efforts, information may be accessible by others. You can help guard against this by never disclosing your password, giving anyone else access to your personal computer, and by ensuring that information you disclose online is transmitted securely.
Third Party Web Sites
Alere cannot control the privacy policies of Web sites you may access through this Site that are not owned or operated by us. In order to determine the privacy practices of those third party Web sites, you must refer to the privacy policies, if any, associated with those Web sites.

If you agree to receive them, we may send you communications, including e-mail communications, regarding products, services, promotions, changes to the Site, or other information. You always have the opportunity to choose not to receive such communications:
a) By following unsubscribe directions stated in the footer of e-mail communications.
b) By contacting Customer Care through the Site.
If you would like to receive this policy in another language, have any questions about this privacy statement, the practices of our Site, or your dealings with our Site, please feel free to contact us by sending an e-mail to email@example.com. A customer care representative will respond to your message within one business day.
Alere reserves the right to make changes to this Policy at any time by posting our new policy on the Site. Modifications to the Policy shall be effective when they are posted on this Site, in which case posting shall constitute notice to you. To the extent we make material changes to the Policy, we will list those sections to which the material changes were made below. Your continued use of the Site following the posting of any such modifications on the Site constitutes your acceptance and agreement to be bound by such modifications. We encourage you to check the Site regularly to see if we have made any modifications to this Policy.
